Save As dialog

Does anyone know how to diplay the attached dialog? This dialog comes up when doing a save/saveas through ODMA.

This is the dialog that allows the user to pick the CheckinDisposition options (NewDocument,NewVersion,ReplaceDocumentOnly,ReplaceOriginal).
