PHP errors out when previewing

Teamsite 6.1 Solaris 9

For some reason I can't get PHP to "preview" or "generate" within forms publishing. Anyone know why the folllowing would create an error?

You'll need to close your iw_pt tag at the beginning. Otherwise, it's expecting strictly XML between your iw_pt tags.

So, start with <iw_pt/>
