Lookup Applet Notintited

Hi All,
We are using worksite mp4 sp1 when we are creating form is forms manager and after then when we are starting the process in worksite mp
we are geeting error message like "lookup Applet not Intited" also some java script error .Please find enclosed her the screen shot also of the same .
