How do you get task comments popup window to appear from a user tasks. Basically, I am looking to capture user comments on approval or rejection of comments. Currently after they select the next task, no window pops up. Is this something that has to be part of an external task?

In 6.x the comments go at the bottom of the task details screen, just above the buttons used for transitioning the task.
I don't believe there is currently any way to make the comments required (might be a good [revised] feature request) - so it's basically a training issue right now since it's fairly easy to mistake the box for comments as just an empty area of the task details screen.
