How to datadeploy a simple replicant

I have a form with just two fields ie.

1. Title
2. Review Reader

These two are grouped into an item and can be replicated togather. As the code shows below.

Can nebody help me out as to how the datadeploy configuration file should be witten and how the database should be designed for this. I want to insert both the values into the database.

This sounds like a "can somebody do my work for me - for free" request.

My standard answer in such cases is RTFM (both the FormsPublisher Developer's Guide and the DataDeploy, and perhaps OpenDeploy, manuals) and to go get training from Interwoven --or-- hire someone with these abilities already.

Perhaps someone else will provide a more detailed response for you.
