Did you checked Published Templates? probably it might not available there?

What you can try, just draft another template and publish it and then try adding it in workspace, hope it will help

Since you saw the template in the previous deploy template dialog, the template should have been available in the system. The error could be something else, please check the log files. If your log files are really big, it might be difficult to locate the correct error. I would recommend to stop the web and cms servers if possible, move the current set of logs into a different directory, restart the servers, reproduce the problem and then look at all the logs. You might notice some other issues that might be contributing to this problem.
