Is there some way to pass custom perl variables from a presentation template, to its included presentation template ?

I want this 'included' PT to act dynamically to behave differently, depending on the flag's value.

Can this be done ?

As far as I know, no it can't be done. Your best bet would be to create a module instead of child/sub pt's. Of course, the module can't access any of the tpl functions directly, you'll need to pass references to all of the functions you want to use to your module.

How about using EA (Extended Attributes) to do this?
If your PT is being invoked for a DCR, then you could possibly set EA on DCR from parent PT by using CLT (Command Line Tool) iwextattr and then in the child PT you could retrieve this EA from DCR by using the same CLT.

You can pass simple Perl scalars or arrays, single DCR values or DCR containers.

Can be a royal pain to get working sometimes though.
