Selecting activity form template in process builder activity

I have a process, XYZ Review, with a manual activity called Review. In this activity I want to specify the form to be used when performing the task. I create a form based on the Document View template, XYZ Document View, where I mapped to my custom type, xyz_document. Next I created a form based on the Activity template, XYZ Document Review. In this form I selected my process, XYZ Review, selected the Review activity, and several of the process variables. I also selected the XYZ Document View to view the package. I installed both forms. Now in process builder, in XYZ Review process's Review activity properties dialog, I don't see my XYZ Document Review form listed. What am I missing?

I managed to get it to work, somehow. It seems that if you create a Task based form template (in Forms Builder) and associate it with a particular process's manual activity, the form value dropdown for activity (on shown on its properties dialog) would update automatically. It doesn't always refresh the dropdown list, even if you click on the refresh button. Maybe you have to close and reopen the process template. Sometimes you have to checkout and then cancel checkout. If the link beween the two is created properly, there should a dm_relation entry named dm_process_form where the child Id is the form object and the parent Id is the process template.
