I am getting an error when an form instance is created.

XFormsTemplateCache construct dynamic models for Sample2[09de75d18000b904]

Required property: sql not found.

DQL Query : select billing_type from sample_document where document_number = '${document_number}'

document_number is the name of the textfield.

Input Parameter : document_number

I am usng the adapter when the input changes (Execute adapter after the input value changes)

Possibly you have configured the initial parameter wrong.Note that the parameter name must be "dql" for dql queries and "sql" for db queries.The video will help you to find what is wrong.
