Form is not binding to the corresponding activity in process builder

Hi,

I am developing an application using xCP platform. I have created a workflow using 3 Manual activities in process builder. I have created 3 forms(XYZForm1,XYZForm2) in forms builder and installed those forms on to the repository and the same forms are attached to the respective activities in workflow. But unfortunately the first form was deleted from the activity-1 some how and it is showing up only 2 forms. I have recreated the same form and installed but still the new form is not visible in the dropdown list except (No Form) ,(Remove Form), XYZForm2, XYZForm3. I have restarted the process builder and forms builder several times but i am unable to find the form there in dropdown list. Can anyone help me in this regard.

That being said - have you added your form as a component to your taskspace application?
