how to add custom applet document viewer form builder

IHi,I have integrate my custom applet doc viewer in taskspace,like daeja or brava,and I am able to choose from configuration tab for format.

Now I have to create my custom Document View template with form builder,and I have to integrate my viewer in the custom form too.

How can I make my custom viewer visible in form builder?
