Problem on Metadata Capture Template CGI Callout

We have a callout button in Metadata Capture Template. By clicking on the button, the calendar shoud be populated. However, it doesn't launch and load up the CGI callout page properly. The problem is that it first loads HTTP 404 Not Found page and after one second, it loads the calendar. In the carlendar page, you can still use right-click to go back to that HTTP 404 Not Found page. The error message in HTTP 404 Not Found page is as follows:

The page cannot be found
The page you are looking for might have been removed, had its name changed, or is temporarily unavailable.

Please try the following:

If you typed the page address in the Address bar, make sure that it is spelled correctly.

Open the localhost home page, and then look for links to the information you want.
Click the Back button to try another link.
Click Search to look for information on the Internet.

I seem to recall that there's some "built-in" HTML page that the callouts try to load first - which, for some reason, did not get shipped with the product.

If you do not have a

iwhome/httpd/iw/blankgrey.html

file - create one with the following contents:

<html><body bgcolor='gray'></body></html>

and see if that resolves the problem.
