How can I add a label to "Allow user to write in an option" field in a dropdown list in Forms Builde

Hi,

First time I am trying to create a form using Forms Builder. There is an option of "Allow user to write in an option" field in a dropdown list. I selected this option, but I do not know how I can lable it? If you have any idea, please give me some detailed steps.

I also have a similar issue of adding lable to the FREE option in "Check Box Group".

Note: Please see the attachment, how I want.

Please share your knowledge and experiences for figuring this out.
