How to delete a form template-giving error

Hi,

I have some problem with my installed Form template. When I try to create an instance (File ->new -> Form) I am getting an error saying "Bad Id Given". Please see attachment. I need to delete it totally from docabse. How can I do this. It is in installed state and I could not open it in Forms Builder to uninstall it. It is also giving error.

Please share your knowledge and experiences for figuring this out.

There should be File->Delete menu item once you open the form and have it uninstalled.
