Log not showing failed deploy

We have had an instance where we deployed a set of files via the Opedeploy GUI using edition comparison from teamsite. Deploying from a unix server to a windows web server running IIS.
The logs say the deployment was completed successfully, with the given number of files. When we check on the web server 1 files is missing. The manifest file includes the missing file and says it is completed, and the receiver logs states the file was received. The file in question is a gif file.
Has any body else experience this problem? Any ideas what is happening?

That's strange... what's the 'reason' and 'action' fields set as in the manifest... can you post the line in the manifest for that file? Are you certain the file is in the source edition?

below is the line from the manifest file:

The file is in the workarea, staging and the edition. The missing file is listed in all the logs as completed, even in the receiver logs as shown below:

Hmm ... any weird properties set on that file? Did other jpg files deploy OK? What's the size of the file... can you post your deployment config file? Very weird to just have one file missing like that, and always the same file... it's got to be sometthing obvious.

As far as we can see there are no strange properties set on this file.

All the other gif deployed with no problem.
I should say, we have tried the deployment again and all the files sent successfully including the missing file. My main concern is that Opendeploy is not logging that one file failed.

Either you're making typos by hand-copying, or you're looking at the wrong information somewhere:

It should be :

The other problem is that you need to capture the logs and manifest from the run in which a file did NOT get deployed.
The manifest is overwritten each time a deployment is made - so unless you capture it when something didn't get deployed you're not really tracking anything.
The log files (src.* and rcv.*) from the run in which a file does not seem to get deployed are also important - these logfiles get appended to, so you have to make sure you're looking at the correct portion of the logfile to make sure you're looking at the right "run".

We went through all the log files (src.* and rcv.*) before any other deployments were made and there is no record of the a failed deploy or error in the logs.

Is this reproducible? Or did you only see this one time? OD is very reliable with recording errors. I haven't seen this type of problem before.
