email notification - sometimes sent and sometimes not

Environment details

Content Server 6.6  (Linux Server)

Workflow Configuration

1. We have a workflow activity with Performer set as "Single User from Group"

2. We have a custom email template in the Notification TAB of that activity

3. We have several activites with same setting as in 1 and 2

Problem

For the same activity, email notification are being sent sometimes and sometimes not to the same user/groups This has been happening only for the past  2 months. No changes were made to the application.

I enabled the tracking to see logs in the BE_dm_eventsender_ebs.log (I hope the name is right). In the logs we find notifications for some and not for the same activity again.

Any idea what could be the problem here.

Try to check java Method Server log beside

also try to check server Email notification troubleshooting guide at the following link

Beside try to make this activity performer is specific user and see how its going to behave.
