Initiate Process Form is Loading

Hi,

I created a Initiate process template for a process and trying to invoke the process using a Action Button from a custom Tab. If i start the process from the default process tab the Initiate process template is loading but If i start the process using the custom action button from a custom tab it is not loading...

Any pointed would be helpful
