Ingest Multiple Assets leaving blank placeholder

When our Asset Manager ingests mutilple assets some of the assets do not ingest. The assets hang in the active job list for hours. When the job gets cancelled there is still a placeholder in Media Bin Explorer for the asset even though the asset did not ingest. Any help is appreciated. Curretly running Media Bin 4.0. Upgrading is not an option. No changes to the system. This just started happening out of nowhere.

What file types are these assets and how are you ingesting them? Through a script or manually?

Are you able to open these files outside of MediaBin in a browser or editing software? Did you try restarting the MediaBin server?

Yes we can open files else where. It just happens on a few files during ingesting. The server was restarted.

Please open a support case. To clarify the white box is not the same as a placeholder image but is what we internally call a "pending" (or less formally "ghost") image. These should never outlast the job they correspond to (i.e., they should either turn into full blown images on success or disappear on failure). The culprit is almost certainly some property of the assets being inserted. We will be able to check if there is an existing patch or whether there needs to be one.
