what's up with generate/regenerate?

I made a change to the tpl and did a preview and saw the change in the preview page.

Then, I tried going to a previouly generated output file, selected it, then Actions->Regenerate Page. The result said that the page was successfully regenerated but the change I made didn't show up.

Then, I tried going to a dcr, selected it, then Actions->Generate Page and chose a file that was previously generated and overwrote it. The changes didn't show up.

Then, I tried to generate to a file with a new name and the changes show up. Why is iwgen/iwregen not invoked in the background in certain situations?

You probably have caching issues. Note that it's not only your browser cache (that too) but possibly your Web Server/App Server cache(s) as well.
If that's the case, you saw "staled" pages rendered from cache although underlying assets were modified
