design problem !! a form with in a form

hi ,

i have this scenario. my web page has a form like the one below( the customer is the one who will enter his/her data not the teamsite editor or author)

Contact Information
*Company name:
*First name :
*Last name :
*Contact phone :
*Mobile phone:
Best time to reach you:
*State Select State :
ZIP Code:

i would apprecite your advice in the best way to approch the design of this DCT.

i am thinking of creating a form with in my form. or just creating an empty field for the teamsite user to add the html code which will generate the form . am i making it too hard for my self here???

i am sure some of you must have done something similar to this.

In the simplest case you do not include any of these in your DCT.
Just generate the whole Form HTML from your presentation template.

In more complex case you can expose Form Labels (if you need them translated
or adjusted in any manner). Then you create "Label" items and so on.

Another aspect to consider is Form's action, that may be allowed for authors
to choose as well using drop-down of available actions for example

Just create the form fields through your DCT and leave rest to tpl file to generate the html form output.
What you can add in the form fields are validation rules drop-down where you can capture if you need numeric values or text value or if it would be an email validation etc..
