SAX Parse:Content is not allowed in prolog

Has anyone seen this DCT problem generated during form-entry or know of a workaround?

com.interwoven.ui.formspub.datacapture.dct.InlineCallout$InlineCalloutException: SAX Parse:Content is not allowed in prolog.
at com.interwoven.ui.formspub.datacapture.dct.InlineCalloutProcessor.evaluateInlineCallouts(InlineCalloutProcessor.java:134)

This happens on an inline command (see below). The command generates a GUID and runs fine from the command line. Would there be something incorrect here or any xml attributes required? Thanks!

First, you've lost item's Instance indication.

Second, if you check DTD you will see that <allowed> and <callout> are the only valid subtags of the <hidden> tag, <default> should not be where you place it.

Finally, Developers Guide states directly (up to TS 6.5 anyway):
   <hidden> Specifies that the data will not be shown in the data capture form. A <hidden> field may receive data from a callout program.
   Note: You cannot currently specify a default value
