With so much useful information it's a wonder you can't figure out what's happening.

first this is not the right form area to post your workflow question. it would be more useful if you post it in the Teamsite workflows.

second thing you need to do is to look at the logs files in teamsite under this folder => iw-home/local/logs

use the debugging features that workflows has such as iw_debug_mode etc..

read the workflow manual => look at the debugging techniques chapter.
