Capturing approve/reject comments in email

Hi,

I remember sometime in the last 2 years stumbling across an example of an email that had a textbox allowing a user to type in approve/reject comments in addition to clicking the approve/reject buttons.

But I can't for the life of me seem to find that. Am I hallucinating? Does anyone know where that example is?
