LDAP Connection Fails with Metastorm BPM Version 9.0 SR2

Issue
An LDAP connection is created in the Process Designer, and an LDAP MBO using the connection is used in a grid on a form. The process is deployed. The form fails to open in the web client and generates the following error:
Failed to execute deployed method 'DataValue', using entity 'Grid1'. Multiple-step OLE DB operation generated errors. Check each OLE DB status value, if available. No work was done.
Resolution
This is a known issue (DEF14420). The following is a workaround.
Access the Metastorm BPM Administrative Tools. Navigate to Connections and remove ;UseDTC=no from the end of Connection Info as shown in the screenshot below. NOTE: If you are using Metastorm BPM 9.0.2 with no hotfix, you will need to install hotfix 1 or 2 in order to see the proper Connection Info.

I'm having this same problem, but when I to the "Connections" item in the Administrative Tools, I do not see the Connection Objects I deployed. Any ideas why? I'm running SR2 + Latest Hotfix.
Also, I should mention when I first click on the "Connections" item in the Admnistrative Tools, I am presented with a Messagebox that reads "Error" in the title and the word "sender" in the message area and an OK button.
