Can you point out to any "Mapping processes 101" document?

Greetings,

I am looking for any documentation that can provide the basic steps, like:

1. Process maps run left to right, top to bottom
2. Incoming arrows go on (top and left?) and outgoing ones go (bottom and right?)
3. Each process should have between 7? and 20? steps (min. and max.)
4. etc.

Appreciate any help with this.
