Issue with radio button

Hi folks, when I open a new form it works perfectly fine. But the problem is when I input data and save that DCT, and when I try to reopen that saved form I see one extra radio button comes from nowhere. I have attached this dct, I will really appreciate if anyone run this DCT and let me know the way of correcting these codes.

Which field ? You have 3 set of radio buttons ? What does the label say ?

Thanks for your quick reply, all of those fields behave the same. I mean all of them get one extra radio button out of nowhere.

Can you post a DCR that was created ? Are you changing the value with FormAPI ?

Basically I am using a callServer to input CDATA inside DCR. Other than that I just open that form and input some data and save that form, then reopen the same form. I attach the DCR . I have attached the perl script too, you just have to hard code that DCR path.

And what makes you think that TS Templates are going to be able to handle that change ? Go figure it is opening up differently. I am surprised that it is opening at all

I input that CDATA right after the value tag. If you open my dcr then you will see. and the other strange thing is it works perfectly for all other DCR's. Only with this DCT i am having hard time. I appreciate your time.
