When trying to create a new form this error comes out, first time i see this error? anyone has an idea?

Chances are, you either have naked & in your DCT or your inline callout(s) generate mal-formed XML that contains naked '&', like Ben&Jerry.
If that's the case modify DCT/inlines to use XML entity instead, Ben&Jerry.
