Dependent Choice Lists for Metadata

Is it possible to create a dependent choice list in the Metadata UI without custom tables and a custom ui?

For example if I have two metadata fields 'State' and 'City' is it possible to only display the City values for the selected State?

What would be the best approach to do this?

We were told this would require what you don't want...custom lists and UI. Not bad in and of itself, but having to go back to IS every time we want to fiddle with the lists was untenable. I'm hoping they make this possible via the fat client at some point, somehow.
