Missing versions?

Hi guys, I have weird bug here.

File A has 440+ versions right now, but user has noticed that we are missing versions 385 to 425. Yes, the number just jumped from 385 (Aug 22 08) to 425 (Sept 23 08). No other versions in between.

I have checked the submit log, and sure enough, I have submission records on that file A between Aug 22 and Sept 23 - lots of them!. But those submissions didn't got versioned as they were supposed to.

Anyone got this problem before? Do you know how I can retrieve back the file?

TS is 6.5
