iwsyncdb-initial creates base tables but fails to deploy DCR

In order to configure TS with OD DAS module I did the following:

Create a datacapture.cfg file
Using OD UI:
Generated a dbschema.cfg
Generated a _dd.cfg

Updated od-home/etc/database.xml for the appropriate db connect strings

executed the following command:

iwsyncdb –initial /my-store/main/WORKAREA/my_workarea category/data-type

INITIAL DB TABLES are created; however the DCR fail to deploy:

Did the additional following steps:

Updated the ODHOME/etc/deploy.cfg and added teamsite.version: 6.7.2 to the iwserver section and restarted the server.

… still getting the same error

Tried to configure the OD DAS Event Server Configuration using the OD UI … I get an apache 500 error

Not Sure of the following:

If the Database server has an OD-receiver installed or need at all for datadeploy
If my _dd.cfg file is actually configured properly

Env:
Win2003 SP2
TeamSite 6.7.2
OpenDeploy 6.2.0.
