CGI script form action not working

Hi,
I have a CGI task as the first task in my workflow. If the starter of the workflow has the editor role in the current branch, the we prompt the user to select yes or no for peer review.

See the attached file for the code section for it which is in my CGI script
When I start the workflow the promp window comes up ok (with the yes and no button). However, when I click on Yes or No, an error shows up (see the attached file for the error).

The issue is with the action="" link for the form. I tried action="/iw-bin/DFAIT_init_approval_wf.ipl". Still not working.

Can anyone help please?

Environment variable 'CONTENT_LENGTH' provides the length of the input submitted by the Form's "POST" Method.

Your code does not Submit anything for the simple reason that it does not contain any Input to submit, thus CONTENT_LENGTH=0.
Note that for the "submit" input Type "Value" attribute is just a button label, it is not passed to the "Action" Script. The Form should
have something to post otherwise why would you need it in the first place?
