Customization of workflow instantiation screen using workflow modeller

Hi All,
I am using workflow modeler and i want to put two radio buttons in my workflow instantiation screen along with a calendar. And also i need some validations on the radio buttons.Can anybody help me out on how to achieve this?

1- generate custom instantiation form using iwmodeldct cmd and see cmds doc to know how the cmd works.

2- you will get a file that will look like the famous datacapture.cfg. if you already know how to develop formpublisher templates, then you know what do to next. the only thing here is that the items in the custom instantiation form can represent workflow variables.
