WFM modeler guide will provide you information on how to create instantiation form. Forms Dev/API guide will provide you with rest of the information.

It is just like adding a field to a DCT.

I am not sure if there is an OOTB calender under iw-bin. I generally re-use the one that is used in rest of the TS UI and I am pretty sure that one is NOT located under iw-bin.

The mention calender script is custom one.It is not a default script.

Search these forums and / or support site and you'll find various custom calendars out there.
