Issue with Teleform Verifier - Error OLEDB 80040154

I am experiencing an issue with Teleform Verifier and seeking assistance. When attempting to launch the program, I encounter an error message stating:

Teleform Verifier - Error OLEDB returned result 80040154 when in function IDataInitialize::GetDataSource. Description: unknown error code (V11.3 11311).

This issue has arisen only with recent installations of Teleform Workstation; previous installations on other machines are functioning correctly.

Environment Details:

Teleform Version: 16.3.1
Server OS: Windows Server 2012
Client OS: Windows 10

Steps Taken for Installation:

  • Updated the operating systems hosting the Teleform server, the MS SQL server, and the client.
  • Mapped the Teleform network drive on the client machine for file access.
  • Configured the necessary permissions for domain users in MS SQL Server to access the Teleform database.
  • I have ensured that all systems are up to date and have attempted basic troubleshooting steps such as reinstalling Teleform and verifying network permissions and connectivity.

I would greatly appreciate any insights, suggestions, or recommendations on how to resolve this error.
