I have a checkbox in the form.
I need to check it on click of a button.
Please tell me the procedure for the same.

is initializeDct() getting called?

You need to debug using a lot of alert statements. Try verifying if the calls are reaching inside SelectAllNW().

What I can see that you don't need () in IWEventRegistry.addFormHandler("onFormInit", initializeDct()).

For now, here's the FormAPI code:

IWDatacapture.getItem("/path/NW").setValue([0,1]);

Include this in your SelectAllNW() function and see if it checks both the check-boxes on clicking the callout button.

P.S. Your DCT code is mal-formed. Use pathid="...."
