[NRTDocument ][Refile ]Database transaction failed; see Administrator

I'm using Interwoven 8.5. I have written some custom code in .NET to upload a document to IManage server from my application.(I'm using COM based model i.e. referencing IManage COM dll's)

However, I'm getting the below exception at the line
imanageDoc.Refile(imanageDoc.Profile, imanageDoc.Security);

Error: "[NRTDocument][Refile]Database transaction failed; see Administrator"

can anyone let me know what should I do in order to avoid this error.
