What are the mistakes?

Please see attached dct in that I am able to do new form entry and able to save dcr.

But when I am trying to edit the saved dcr it is empty.

Please suggest what mistakes I did in dct.

The tags are all mixed up of xml and iwov type. Either user xml or iwov, both not at the same type.
