Metadata form not working whenever creating new DCRs

Hi,

I have an issue with metadata form on TS 6.7.2 sp3.

after filling entries in new DCR it showing blank metadata form instead of metadata fields.

but for the existing DCRs, i can go to TAG UI and see metadata fields.

This issue happening on QA box. we have similar functionality on Prod and it's working as expected.

I verified metadata-rules.cfg and it looks good.

Can anyone please suggest me the possible reason for this issue?

All sorts of "ideas" come to mind:

There are probably a number of other things that could be the source of the problem, but we don't have access to your system, logs, config files, or scripts - so you have to do a lot more detective work on your end before we can really provide any assistance.
