Which DTD are you referring to from the FormAPI PDF? There are several shown there.
The init() function is named by convention, you could name it harry() if you wanted to, as long as you use the proper FormHandler directive to call it.

This DTD belongs to one of the FormAPI Manual Samples. It defines certain DCT to enter "basic job posting".
In other words, that's a DTD to illustrate sample XML structure that uses FormAPI.

As far as FormAPI usage itself is concerned you can safely ignore it. FormAPI is essentially a Java Script,
it neither has nor needs any DTD.
