Search retruning old versions

We are using worksitemp 4.1 HFB7. When users search, search is returning old versions rather than new one.

Thanks for reply. We have been working with vendor on this issue. The issue is inso is unable to process some files due to that Indexer is hung. We are seeing around 50k files not indexed?
