HTML Source View Editor Error

Environment: Teamsite 6.5 SP3, Windows 2003 SP1

When I tried to view the HTML view of my content in VFE, I got below error:

"The editor was not able to create the HTML source View Editor."

Any clue what do I do to get rid off from this error?

What is the Client OS? Is it by chance Vista? What is the browser?
Does it happen to all users or just some of them?
