what od you mean by nested?

you cannot nest fixed layouts.

I have an outstanding issue for the past 3+ months with support asking for any information on the format/layout/dtd/tags/attributes/... anything for Page Layouts. They have gotten back to me a number of times with "We're having trouble locating any information on this."

If anyone can shed some light (a DTD would be fantastic) on what options are available for page-layouts I would really appreciate it.

What specifically you need from custom layout perspective, i dont have any information about DTD but may answer specific queries.

Which I believe should give me two side by side areas to put components. When I put a simple text box (OOTB component) in each, they end up above/below each other and not side by side - because the generated HTML has the width of the whole row as 676 and not 930.

Remove width from Column. Area width is autmatically picked up by Columns and rows. Also IDs assigned to Area, Column and Row are same, make them different.

what have you defined in classes herobox and heroImages? Although above Layout code is not perfect but it works in my environment and shows components side by side. So my first guess would be "herobox and heroImages" classes are doing something. Share the definition of these classes

Have you tried the percentage in the width?
Have you applied right css class and defined correctly

My issue is that my changes are not being reflected in Preview (or on our web servers). The changes are appearing in Edit mode, but no where else.

It looks like something is not being deployed or a caching issues (we rebooted the server, so I ruled out caching)

Any guess?

Fixed layout (nested or not) should just determine your html structure (divs and div ids), CSS should be positioning the divs in the page. If you are sure you have the correct style, use firebug to determine what overwrites your css in sitepublisher, that should help you resolve your issue.
