Files are not getting attached to the workflow task

Hi,
We are trying to attach the files to a task in the workflow and we are using task->AddFile($file);.But it is returning the error code of 256.We also tried running via iwaddtaskfile CLT,it works fine in the putty.Only when it runs through the workfow the above error is thrown.The owner of the task is root.

Arguments passed to the add task file is follows

/accounts/100/reports/M/N101666_index.html
/accounts/100/reports/M/index.html
/accounts/100/reports/A/index.html
/accounts/100/reports/A/N101665_index.html

Could anyone help to know why the error code 256 is thrown.Thanks in advance.
