SitePublisher .page and SSI

I have a SitePublisher page that is reference components for header and footer that are generating include files. In the preview mode I have written an external that brings in the content from the include files and it renders correctly. Also when I generate an HTML file the include reference is working as I have configured IIS to render include for HTML files.

However when I create a link in the content and reference a page the link is working correctly and will take me to the page but the header and footer include files are not rendered. If setup the .page extension for SSI like I did for HTML the include works but the page will not look correct with styling.

How can I make the include files work with .page.

The only difference I can infer between the working and not-working states is that the URL might be different?

The web server shouldn't care where you came from. As long as the URL is the same whether you typed it in or linked to it, the page should come out exactly the same. Are they?
