CSV file format for Generating a Report with Parent Child Relationships

I need to generate a report that shows a praent - child relationships. Ex: <br />
<strong class='bbc'>Please note that the report mock-up did not retain the white spaces when posted</strong><br />

I have been playing round with multiple csv file formats which I have attached the csv file formats I have been playing with. Our code will generate the csv file, we just need to know the format it needs to be in, in order to create the parent - child report shown above. <br />

I have been playing with Joined data sets to try to separate the data so that I can use table groupings with inner table groupings. to try to create the above look and feel for the report. I have not been successful.<br />

I am using BIRT version 2.2.2<br />
