To manage the site uses actuate viewer.

But the problem has occurred.

pdf convert to pdf in actuate view from the input tag will disappear.

Client side script and HTML in a BIRT report only work in HTML output. Are you wanting these fields to be interactive in the report or will you be printing them to be filled out? If they are being printed you might be able to check the output type and drop the text elements and place images for your radio buttons and text fields.
