Does anyone know if there is log file in which any errors generated by FormAPI are sent to.
The events on FormAPI suddenly stopped working and we have no clue why.

This is what I have on iw.cfg
[teamsite_templating]
# use_java_ui=false
# data_root=templatedata
enable_formapi=true
has not been disabled.

Typically if there is an error in the beginning seems like nothing else is work. Another issue is a mis named DCT field.

Yes the error FormAPI error handling approach is described in the doco:
Wherever possible, of course, FormAPI handles faulty input gracefully (usually, by doing nothing),
This can make debugging your problem somewhat problematic.
If you are using IE you may get the icon in your status bar indicating a script error has occurred. Double-clicking on this may provide some guidance.
If you have Microsoft Visual Studio installed you can probably even get improved debugging information.
Failing that; the method I have adopted in extreme cases is to remove all FormAPI and then reinsert portions, test, insert some more -- until you can localise your error. It may be something as simple as a missing closing bracket
