files not showing up in Work in Progress section -

The user went into a form and clicked the Generate button without Saving it first, even though she technically didn't have anything to save (she was just regenerating because the page was linked to another form entry file that had been modified). She clicked "Next" and then chose to Keep as Work in Progress. When she went back to the CC home page, the regenerated file didn't appear in Work in Progress (even after refreshing).

Any idea on what could be causing this???
