VisualAnnotate - Can't annotate the HTML file

In a workflow/job, if an author (usertask) happens to go into Review mode of the HTML file (by clicking either SCE tab or VisualAnnotate Review button), the subsequent reviewer (usertask) can't seem to annotate the HTML file. The VisualAnnotate bar will indicate that the file is still being annotated by someone else.
I have used the variable in the first usertask (<variable key="author_task" value="true"/>) to indicate that it's an author task. However, it seems that it's not making any difference (I've tried setting it to "false" also).
Any suggestion to remedy this issue? Thank you.

In any given VisualAnnotate job, the only user who can annotate is the owner of the active task in a serial workflow.

First, check that the reviewer owns the active task. Also, make sure that the author and the reviewer are not the same user.

Out of curiosity, are you using VisualAnnotate on TeamSite 6.0 or TeamSite 5.5.2?
