We have W2K TS 6.0L installed, and there's a directory

iw-home\httpd\webapps\content_center\vannotate

What is this and how did it get here? To use VisualAnnotate, don't I have to download and install something?

What's the system locale? VA is installed on English locale as part of TeamSite 6.0.0L.

System locale is English.

Attached is dir listing from server. Is this complete? How can I tell if we have full VisAnnotate install?

Yes. VA server is fully installed with TeamSite 6.0+. When you attempt to use the toolbar, it will get installed.
